I acknowledge that during the course of performing my duties as a Nominator of candidates for board membership on the West End Opportunity Partnership Board (“The Partnership”), I may have access to, use, or disclose confidential personal information regarding applicants, including without limitation their address, contact information, birth date and other identifying information disclosed by applicant during the application process (the “Confidential Information”). I hereby agree to handle such information in a confidential manner at all times during and after my service as a Nominator and commit to the following obligations:

• A. I will use and disclose Confidential Information only in connection with and for the purpose of performing my assigned duties as a Nominator for The Partnership.
• B. I will request, obtain or communicate Confidential Information only as necessary to perform the Nominator duties for which I’ve volunteered.

I understand and agree that my failure to fulfill any of the obligations set forth in this Agreement could harm an applicant and/or violate their privacy. I will fulfill my duties with respect to confidentiality and privacy.
